Hello out there. I hope someone can help me. After years of getting successful Botox injections, they stopped working. Seven months ago I had my left vocal chord severed. The good news is that, while my voice was rather weak, it was audible and I didn't have undo strain or spasms in my voice production. Unfortunately, three months post surgery my "false vocal chords" began to take over. Even with speech therapy my voice continues to become more strained and difficult to understand. It takes half the muscles in my body to squeeze out a sentence. I am feeling increasingly fatigued and frightened. I don't know anyone else who has had this side effect. Do any of you have any ideas or know anyone who might? I am in the process of contacting the surgeon (Dr. Singer) and Dr. Izdepski. I would very much appreciate your support and guidance in this matter. Thank you! Marianne |

Marianne - I certain feel sorry for your situation and can understand your problems. There are a few men on this BBS who had the SLAD surgery at UCLA who experienced unexpected results, i.e. we did not get anything near a normal voice back. What sort of surgery did you have performed? I'd be interested in more details as this might help others come forth with helpful information. John |

Hi Marianne, I wrote you around a year or two ago (more or less??), based on your post to the BB. Tried to assist you at that time, as I had also had that surgery and you are in my local area. Sorry to hear this latest. What you describe is similar to what I'm experiencing recently (false cords taking over - laying over the true cords so that not even a flexible laryngoscope or videolaryngostroboscopy can tell what's really going on). However this situation did not happen for well over 6 years post my RLN surgery. I functioned semi-well for 6 years (albeit low-volume over noise and breathing issues). At this point, my team of doctors (also including Izdebski) consider my case *extremely unique.* One of a kind. I do not sound to anybody as if I have the extremely severe case of "classic spasmy/straining AD/SD" that I had prior to surgery. No vocal spasms since surgery but I often sound as if I'm having major respiratory failure and other forms of vocal break-down (gravelly, growelly, raspy, hoarse, falsetto, breathy, constricted airway, etc.). Serious side-effects (breathing problems since surgery, and now this new stuff) which, often, are worse than the spasms/tremors were, over several years. Any of this sound familiar? Sometimes one wonders whether the side-effects of a (*any*) treatment are worse than the original condition. I've wondered that with having the same surgery you had, but had zero choice at the time. Dear Lynne, Thank you so much for your prompt reply. Sorry for the delay in answering. I have been away on vacation. I am so sorry to read of your struggles with SD and the side effects of surgery. I wonder if the procedure that Narelle Lehane discussed in her posting would be any benefit to you. I have emailed the info to Dr. Singer and will set up an appointment to explore my options. As you can well imagine, I am very wary of any further surgical "solutions." I am going to try to make it to the January support meeting. Hope to see you there. Again, many thanks for your support. I had the thyroplasty type 2 and had MTD on top of that (was there before but hard to second guess that). My ENT lasered back my false cords so they can't touch each other - a relatively easy technique with little pain. I was too stressed out for speech therapy to help - a vicious cycle - but this has broken that and I have a happy and relaxed throat. So I am not stressed, my muscles have relaxed, so I can speak so much better and can exercise without hurting myself from being so tense. It sounds like your case is very similar to mine - and the more stressed you get the worse it gets. It was my speech therapist who suggested the laser technique to my surgeon, they work together, as she knew it was debilitating and also devastating emotionally after having had the surgery successfully. Talk to your docs, they may know about it. My ENT is Mr Neville Minnis whose credentials and contact deatils are on this BB somewhere - if your docs want to contact him he'd be happy to help. Hi! I'm from the Chicago area and I think the surgery you are talking about is the original surgery that started in the 80's.A women in The Chicago support group(Cathy Jacox ,I think thats how you spell her name) had surgery with both sides cut and she couldn't talk at all. Just a whisper and somehow I don't know the specifics but Dr Bastian of Loyola Health clinic in Maywood Illinois did some type of surgery to reverse the effect,This was a few years ago. I don't know if this is similar to your problem but thought I would mention it. |

Dear Narelle, Thank you so much for your support and for the information you sent me. Sorry for the delay in responding to you. I have been out on vacation for two weeks. I emailed my surgeon the info you shared. Could you please elaborate more about the surgery you had and what your false vocal chords were doing? I have wondered if there was some way that they could be "quieted." Currently, they have taken over the show and, as you know, even with speech therapy, it is a constant battle to squeeze out voice production. I am reticent to do any more surgical procedures but your email was the most hopeful news I've had in a long time. Thanks again, Marianne |
