Hi everyone, Im 27 and been dealing with ABSD for 2 years now. I've had multiple botox injections with varied success. Starting in Oct I now get my botox shots in the OR. First one was pretty good, then just last week I got another one at a higher dose (around 15 units)-yes I take a lot of botox!). Well 3 days after, my voice went from normal to soft, then to a whisper. And sometimes when I drink water I choke on it. I can't yell if I tried! I can't answer the phone at work at all now. I went to my doc yesterday for an answer and he thinks that when he injected my PCA muscle (AB), a few days later some of the botox dispersed into the LCA muscle (AD)because it's right next to it. So basically I have botox in both muscles, and the result is a breathy whisper voice with fewer abductor breaks. It's awful! He said "Well it's still early, I think these side effects will go away and you'll have a strong voice for a few months". Problem is..he didn't tell me how long this breathy/whisper voice will last! For those of you with ADSD, how long does the whisper voice last? It's just really embarrassing and frustrating..I mean my adductor muscles are weakened so my vocal cords are apart, but my abductor muscle was injected to keep them together! It's like cancelling each other out! ARG!!! |

You have just learned what those of us with lots of Botox experience already know. There a a lot of variables in injecting Botox including the skill of the doctor doing the injections. It is very difficult to hit the correct muscle for ABSD (I have ABSD) and if you get a doctor who can do so time after time, then let everyone know as this would be amazing. I have had injections from four different doctors at dosages from .25 up to 10 without much success. The doctors were all at highly respected hospitals or clinics. It takes lots of injections (read patience on your part) in order to get the right amount in the right spot. Have patience. It is not easy running into the problems that you are having but the problems will lessen as time goes on. The important thing is for you to keep documentation of the process. If you aren't already doing so, then download a form off the NSDA website to track your progress. Good luck to you. John |

Hi John, I guess I have to consider myself fortunate in that I do receive a benefit for my AB after each of my bilateral injections. My dosage is generally around 3.6 units per side depending on how much residual there appears to be from the last injection. I didn't realize that others, from what you are saying, apparently do not get consistent results. Best,

After my last botox injection (almost 2 years ago), loss of voice lasted a month. The time before,it had only lasted 3-4 days. Because I have to use my voice at work, that wasn't very good! In the meantime, I've tried speech therapy, massage, breathing exercises, etc., which have been somewhat helpful, but now that my voice is really getting worse, I've scheduled another botox injection, and am hopeful that it will be better this time. My voice is getting so weak now, that a whispery voice for a month can't be any worse, and the months of a good voice will be worth it. - Sue |

The Botox injections have I have had for abductor SD from two different physicians were not very successful. It is more difficult to reach the target muscles for the abductor SD population. Just part of being the "rarest of the rare" in our neurological voice disorder community. Mary Bifaro

Hi, It is certainly possible for the botox to diffuse to other muscles. Even with a laryngoscope-guided needle, it is hard to get the botox to exactly the same part of the muscle each time. I have never had the experience that you are describing however. My side effects have always been slight shortening of breath and/or some swallowing difficulty. I hardly get either of these anymore. Hopefully your voice will return with a good benefit from the injection. Best wishes. Keith |
