I began a thread about a year ago telling of my experience using Delsym cough medicine and I wanted to give an update. After taking it for about 1 1/2 years with excellent results I ran out and did not get to the store right away to get more. That was right around Thanksgiving and I have only taken it once since then. When I went to my doctor he commented that my voice seemed better (that was before I told him that I wasn't taking the medicine anymore) and when I asked the speech therapist that I work with what she thought, she said she couldn't tell any difference. So far, so good. I can sometimes still feel the tightness in my throat and sometimes hear a slight tremor. This is usually at the end of the day or during a meeting when I have to talk, but I'd say that my voice is probably 90% normal now. I don't know if it will last, but I can always go back to the cough medicine if I have to. I know that this doesn't work for everyone, but wanted to pass on my success story and let everyone know that there is hope. I only took one dose once a day. It did take several days to be fully effective although I did notice improvement after only a day or two. My doctor actually encouraged me to try taking more, but I felt that I was getting good results with the one dose. (That's why I used Delsym...it's good for 12 hours, other cough medicines worked including the capsules from Robitussin, but you have to take more doses.) If you check the archives you can find more information from past discussions. This summer while on vacation I forgot to take it for several days, but I did notice that my voice got worse after 2 or 4 days, but so far it's been about 1 1/2 months this time and my voice is still good.

Do you have SD or MTD? I tried it (see my thread on Dextromethorpham HBr) for MTD and it did nothing. I was using the Robitussin capsules...I believe 25mg. each. I took four per day (bottle said not to exceed more than 6 (???) in a 24 hour period, I believe. If you're taking the full adult dose of Delsym, it looks like that is 60mg. per dose. Now I'm wondering if I should retry using Delsym at the higher dosage????

Dextromethorpham shouldn't help someone with MTD at all. It is classified neurochemically as an NMDA receptor antagonist and acts centrally (on the brain) as opposed to locally (on the respiratory tract), which is why it's sometimes effective for those with SD, which is a neurological disorder.

The best treatment for MTD is speech therapy, maybe some muscle relaxants, and practicing relaxation techniques, like massage, neck and shoulder exercises, acupuncture, etc. Although it's still a challenge, those with MTD are more fortunate than those with SD as you can eventually get your normal voice back.

Thanks Lori! I am seeing a really good otolaryngologist and she has me doing massage, relaxation techniques and vocal exercises to "find" my voice again. It is really strangled and has progressively worsened over about the last 15 years. She is certain it is MTD but cannot rule out a minor SD component. I won't try the Delsym. It's just that a bad cold seems to give me a "voice" but it's likely due to the cold's effect rather than the medication.

In addition to the otolaryngologist (ENT doctor), you should also be seeing a really good speech therapist who has extensive experience treating those with MTD and SD. You can find a good list on the NSDA website at www.dysphonia.org (there's also a link at the top of this bulletin board), then click on the "Healthcare Referrals" link on the top menu.

Thanks again Lori. She is a Senior Speech-Language Pathologist, MS, CCC-SLP at Loyola University Medical Center. I was referred to her by an ENT doc. I'm very fortunate to have been referred to her. I think I'm on the right track.

- ok, so here's the thing. dextromethorphan (Hbr). I had originally tried Delsym cough syryp quite some time ago and gave it up after a couple of shots, couldn't handle the taste on a daily basis. Anyway, few days ago I bought some robitussin gel-caps, ya'll know how it is, try anything. Lo and behold, it seems to help some. I actually had a conversation with a co-worker this morning and I was mostly intelligible. While by no means a perfect voice, it was a blessing to be able to actually have a back and forth with another human. Also, on the phone yesterday with my sister, she could actually understand me and phones are the worst, ya know?

I've been taking two about every 4-6 hours, although the directions say 6-8 hours. I figger after taking all sorts of Rx medications; that taking an OTC at more than directed isn't going to hurt. So I'm experimenting. I pray that it keeps helping as it seems to be.

I have the more common one - AD? I get them confused. Not the breathy type but the strangled voice type.

Another update update on dextramathorphan. it's like...OMG! ok ok... maybe not OMG! however, I will go as far as to say, it definitely does make a difference. Even though far from perfect, my voice is also far from as bad as it was. Difference is very noticible on the telephone and thats saying something. I'm taking 3 robitussin cough gel caps in the morning and another 3 about 5-6 hours later. I actually bought Walgreen's store brand the second time I purchashed and see no difference. Even when my voice "goes under", it still isn't as nearly as much an effort to get words out, not efortless but not nearly as tiring, for sure.

This is Chaanz coming at you from a pretty good mood and cuatiously optimistic.

Hi Chaanz,

Could you tell me how you figured out what dosage level worked. I’ve been trying the Gel caps (15mg) but have only been taking 1 “cap” three times per day. I think they help me too, but I cannot tell for sure unless I stop taking them. I have had two botox treatments so far and I really cannot stand the procedure.

Hi - In answer to your question - I didn't figure out what doseage to take - I mentioned earlier that I was experimenting to see what might work and for me, taking the recommended doseage at the recommended time didn't do much, so I upped the dose to 3 caps about 5 hours apart. I'm having fairly good results with it and I'm hesitant to take more than 3 at one time. I'll probably ask my doc next time I see him about ramifications (if any) of higher doses. The difference isn't night/day but it is enough so that my self confidence has risen some and I feel a whole lot better about myself. Botox is no longer an option for me since thyroplasty (and this is a lot less expensive :)

Keep in mind, what works for one may not work for another, but I wish you good luck and keep us up to speed on how you do with the cough gels.

It's so great to hear that the cough medicine is helping other people too! I tried the gel caps too, but I could go longer between doses with the Delsym, so it was worth it to me to tolerate the liquid medicine. To each his own! I'm anxious to hear how others do long term. I still haven't taken anything since Thanksgiving and my voice is still doing good. I do occasionally notice a tremor or a "catch" in it, especially when stressed, but I just take a deep breath and relax and that seems to work. It's good to know that if it starts getting worse again I have a relatively painless and inexpensive fix for it. Hope the dextromethorphan keeps working for you!!!
