Upcoming knee replacement!

I know this isn't about knees, but..........I'm having a total knee replacement done the end of February. Normally when one has surgery they are "intubated" after they're "out" with an anesthetic for the length of the operation. I just had my Botox last week and it generally lasts 3 months.....I am concerned about how the intubation will go what with my vocal cords being, I think, paralyzed until Botox wears off. Does anyone have any idea/knowledge about this? I will, of course, talk to the doctor about this next time I see him, but wondered if any of you have any prior experience with being intubated for surgery??? Just curious. Thanks, if you have any info!

I am an Anesthesiologist with SD. In regards to your upcoming total knee replacement, you will be intubated if you have a general anesthetic. You should inform your Anesthesiologist about your SD and the Botox treatments. You might want to remind him or her that what you have is a dystonia. At least theoretically, there are some anti-nausea medications that should be avoided as they may cause dystonic symptoms. You can be safely intubated for the surgery but it would be helpful for the Anesthesiologist to appreciate that your vocal cords are partially paralyzed by Botox. I personally had a general anesthetic that required intubation a few years ago without any problems. You should also know that the surgery can also be performed under a spinal or epidural anesthetic. Your trachea would not be intubated with these anesthetics. I recommend that you fully discuss your entire medical history and your concerns with the Anesthesiologist. Together, you should be able to come up with an Anesthetic plan to safely meet your needs. Good Luck!

Hi there,

I have an op. coming up, and am booked in for a botox injection for my AB SD, 2 days before the operation. I did talk to my Consultants secretary who spoke to the registrar who said there shouldn't be any problems, but on reflection I think I didn't mention that I have abductor SD as opposed to Adductor. I also have asthma (although not severe) and I have had problems after botox with my breathing. So, I think I need to go back and talk to the Doctor himself, and see what he says before I have the next botox injection.

I'll let you know what he says, good luck with your knee replacement op.

Hi...

About a year ago I had a major surgery and was concerned about intubation because I've heard that it can occasionally (rarely) be hard on people with SD (and can even be a possible trigger for SD symptoms appearing for the first time in people.)

My surgery had to be done, however, so just before surgery, I spent some time talking with my anesthesiologist and my fears about intubation making my SD worse. He had actually heard of SD but had not heard about the problems with intubation. He spent some time researching SD (so that was good, as now another person in the medical field knows a LOT about SD. ) In the end he could only promise to move very carefully and slowly as he did the intubation (and later removed it.) I ended up having no added effects from the intubation (and my surgery was a couple of hours long.)

Good luck to you, and I hope all goes well. I would take the time to talk with your anesthesiologist about your concerns, though.

Hi there,

I spoke with my speech pathoologist who works with the Doctor that administers my botox injections re: an operation I have coming up in Feb. where I will be having general anesetic. She basically said that 'technically' there shouldn't be any problems, but on balance she felt that I should cancel my botox injection that was booked 2 days before the operation.
Just to re-cap I do have abductor SD which can effect breathing.
