method of botox injection

I have been receiving botox injections for the last 3 years for AD SD with inconsistent results. Today I consulted with another ENT and he uses a method to inject that I'm unfamiliar with. He uses a scope through the nose that contains a needle. I've always been injected through the neck and had become comfortable with this procedure. I'm wondering if anyone who has had both procedures could compare/contrast the experiences. I appreciate any information about this topic. Thank you ! Jill Newhouse

I have had botox administered both through the neck and with a scope down my nose. I feel that both methods can be accurate. If they are going down the nose, you might want to be sprayed with some lidocaine to help with the gag reflex. I use this method now and it works...

hi...i also have adsd...My first 2 botox injections were done through the neck needle only...I had mixed results. its my understanding that the scope is used primarily to see the cords. It allows the ent to target the cords better..I had the scope my last 2 injections..it works wonders..I havent heard of the needle being on the scope. Is this a new procedure?

I have not heard of this either and suspect it is in error. My doctor uses the scope while administering the botox, but only as a way to see where the needle is going once it passes through the cartilege (frontal injections). Obviously, the needle would need to be one that retracts fully into the tube...otherwise it'd sure hurt trying to make that turn down through the nasal passages. I'm wondering if this was reported accurately?? I just can't imagine this...think how long that needle would have to be.....yikes...

Sorry, perhaps my post didn't contain enough info. Just to clarify... this procedure to inject the botox involves a scope containing a retractable needle (they demonstrated it for me) It's similar to the scope that is commonly used by ENTs to look at the cords, but is slightly larger in diameter (I'm assuming because it contains the needle??). This particular ENT has used different techniques to inject the botox, but is most comfortable with the results when it's administered via the scope through the nose. I think I'm just quite nervous about a new procedure (and new doctor). I am scheduled to have this done in a few weeks. This Dr. also injects some patients in the operating room at the hospital if they have difficulty with the procedure and require more intensive anesthesia. I think it was this knowledge that concerned me and I'm just very anxious.

I have both methods of injection. (1) this was a scope through the nose, down my throat, with a needle being inserted outside of the neck to the vocal cords. (2) with a needle in the scope.

I have no preference on either. they were about the same for me and not a problem.

I am very interested in learning more about this method of injection. I have only received injections through the neck and have had sporadic success. I was wondering if those who have had similar results from both methods, had a scope placement for both injections? My ENT does injections through the neck, but with no scope placement. I am wondering how much this visual adds to the success rates for injections? I would apreciate any feedback.

Thank you so much!

Michelle, this is just a personal opinion on this. First, the needle through the throat is basically the doctor listening to voice noises to help him to determine the right spot. In my case, this was a hit and miss situation since they could not really see the exact area and it was done by feel and sound.
My current doctor uses a scope and needle and hits the right place every time, so this is good for me

I just had the new procedure done today and will anxiously await the results. It was a completely different sensation during the injection, but not painful and that was what I was quite concerned about. I also had the option of watching the injection on the monitor but preferred to close my eyes; maybe next time! It makes sense to me that this method may be more precise, because the Dr. is actually seeing the injection into the cord via the monitor. My Dr. also mentioned a "puffing up" that could be seen as the botox entered the vocal cord. I'm keeping my fingers crossed...
