Hello. I had my first successful BOTOX injections on 12 March of this year and now am experiencing the first signs of SD retuning. I'm curious as to whether the average patient experiences a rapid return of SD symptoms from this point and the voice quality rapidly declines, or the BOTOX benefits make a gradual exit and there still is good voice for some time. My MD schedules the BOTOX clinics for the second Wednesday of each month and I'm wondering if I should my my appointment for June or July. Also, can you tell me if it's routine to wait until your SD symptoms fully return before you get another treatment, or should you get your next injections before the effects of the last treatment fully wear off? Any information that can be provided regarding these issues would be greatly appreciated! Yours spasmodically, Tom Daley

Tom: I had my first Botox injection on January 12 of this year, and by the end of February I noticed that I was once again struggling to talk and make myself heard. I did not have any side effects with this first injection, which was 2 units bilaterally. My doctor told me that when I first start noticing a deterioration in my voice and difficulty talking, that is the time to schedule another injection. Living in Alaska and woring full-time, nd the expense of traveling out of state, I could not get down south until May. I had my last injection on May 2, and this time I had the breathy voice for almost three weeks, and have just now been able to swallow liquids without some choking. I received 3 units bilaterally this time. My voice is very clear and I am hoping that since I had almost a month of side effects, that my good voice will last much longer. Take care and think positive thoughts. Pat Odell, AD/SD |

Hi Tom,
Congratulations on having a successful BOTOX treatment. I think that most of us experience a gradual return of symptoms as the BOTOX wears off. For me the shots last about six months. During the final month I can start to feel the symptoms return, and if I let it go longer than six months I'm pretty much back to baseline. A lot of folks only get two or three months of voice benefit from an injection, and a few people I know get as much as eight or nine months. Whether you wait until your symptoms fully return is up to you. At this point I schedule my injections in advance for the time I expect my symptoms will be just starting to return. I know a couple of people who get injected with a low dose every couple of months and keep a fairly continuous good voice. But if you are just getting started with BOTOX it is probably best to let your symptoms return before getting re-injected. The response you get from the second injection may not be exactly the same as the first one. After you get more experience you will have a better idea about how long to wait. One caution is that you should not get injected more often than six weeks, that can cause an "innoculation" effect which can lead to immunity to the toxin. Best,

Hello Tom,
I too have only had two injections. My first one was 1.25 in each chord and worked somewhat successfully. I went back Dec. 13 of 2002 to have my second injection. My doctor shot the left chord but had difficulty finding what he called an "active spot" in the right one. He said this was because of residual botox left from the first injection. At any rate, this second time around with botox has been an excellent one. By the same evening of the second inject (1.80 per chord) my voice had become whispery as compared to about four or five days after the first inject. This second injection has lasted almost 6 months now. I can feel catches though coming back with my voice, but I want to wait until it gets a little worse before going back. My doctor also thinks that placement of the shot is very important and trying to keep the botox dosage as low as possible. But it is different for everyone. So far this has worked for me. Jana |

Hi Tom, I was just recently diagnosed and had my first injection about 3 1/2 weeks ago. I can hear a difference but not as much as I would like. They said it would take a few shots to get it regulated, as everyone reacts differently to botox. They told me to make an appointment for another shot when I felt the effects of the previous one wearing off. I have sd/ab but not a severe case. I was 14 when my voice started giving out on me and 39 when I was finally diagnosed. Good luck with your next shot. Debra |

Hi Tom, My botox injections lasted about a month, but it seems like most have longer lasting results than I. Being your first one it will take a few more injection to find out what dosage will give you the most benificial results. I found once my voice felt as if it was going bad, it was gone. There was no gradual delay in between that time. I wouldn't wait to long to make your appointment. They generally like to have at least 4 weeks in between injections. So don't wait to long, or you'll be back to suffering in between appts.
Good Luck, Jennette |
